Salvia divinorum and persistent psychosis

From the American Journal of Psychiatry letters to the editor:

To The Editor: Salvia divinorum (salvia) is a sage plant that is easily obtained in the United States. Its active ingredient, Salvinorin A, is a novel and highly selective pure kappa opiate receptor agonist with rapid onset and powerful hallucinogenic properties (1). Salvia has become increasingly popular as a drug of abuse when smoked. No long-term negative outcomes have been reported from the use of salvia. We present a case in which salvia precipitated persistent psychosis.

"Mr. J" was a 21-year-old man with no family or personal psychiatric history or laboratory abnormalities. He was reported to have normal social interactions, behavior, and cognitive skills. He was transferred to our psychiatric unit for acute psychosis and paranoia, which occurred shortly after smoking salvia. In transport, the patient became suspicious and attempted to jump from the vehicle. Upon presentation, he demonstrated echolalia, paranoia, flight of ideas, and psychomotor agitation. The patient remained agitated for the first 2 days of hospitalization. He attempted to barricade himself in his room. Risperidone (3 mg by mouth/three times per day) was administered, and the patient was eventually stabilized. The dosage, however, resulted in the parkisonian features of rigidity, bradykinesia, and masked facies.

Mr. J was transferred to the chemical dependency unit for further treatment. He was stabilized, and treatment with risperidone was slowly tapered. During the taper from risperidone, the patient continued to improve and manifested better insight and logical thought processes. He participated in group therapy and interacted with peers, and the parkinsonian features subsided. One day after risperidone was withdrawn, the patient’s symptoms abruptly returned. He became agitated, paranoid, and aggressive and believed he was able to project and receive thoughts. He returned to the inpatient psychiatry unit where risperidone (3 mg by mouth/twice daily) was reinstated. He was once again stabilized and transferred to the referring psychiatric facility for further treatment. At the 4-month follow-up, the patient exhibited no perceptible improvement.

To the best of our knowledge, this is the first reported case of a persistent negative outcome from the use of salvia. We suspect that our patient was genetically predisposed to schizophrenia, and salvia precipitated the clinical manifestations. This may relate to the drug’s ability to influence dopamine levels in the brain and potentiate plastic changes in frontal lobe networks (3).

Previous studies have cited salvia as a potential treatment option for CNS illnesses (2). This is surprising, considering the paucity of research regarding its efficacy. Salvia’s ability to increase dopamine levels in the nucleus accumbens increases its potential for dependence. Consistently, patients at our clinic who have used the drug report that its psychological effects are abrupt and frightening. Clinicians should be aware that salvia use can be associated with psychiatric illness.

Many things can trigger peoples potential schizophrenia, but it does not surprise me at all that Salvia does.

I myself once was psychotic for almost 2 weeks after the most intense trip on it that I ever had. I had been taken to an infinite black abyss and thought that I was there for an eternity. When I came back I thought that all my memories were being implanted in my brain and that they were never real to begin with, that everything in my life was just some kind of hologram in front of the black void. For two weeks after I kept thinking that the void was coming after me to suck me back out of the hologram world, that everyone and everything was just a hologram, and that everything I had ever cared about was all part of a delusion.

The feeling faded after 2 weeks, and I have done lower doses since that have not had nearly such negative effects.

One of the problems with it is that it is available in concentrations far more potent than found in nature. The Mazatecs in mexico also did not smoke it, but chewed the leaves which lasted longer, but had effects that were of a less violent intensity.

I don't think that it has nearly the same abuse potential as most drugs due to its extreme and often unpleasant nature, but there will always be those who abuse what most people wont.

I think psychedelics and dissociatives in general should not be illegal, but should be regulated and controlled.

Our society requires screenings, training, and background checks for people to buy guns, I think something similar should be in place for psychedelics.

I will say though, I think guns are a far bigger threat to our society than psychedelics are, I don't see how they can be legal while psycho-actives are not.

Alcohol is legal, even though it causes people everywhere to be violent and dangerous every weekend, physically damages peoples bodies over time, is physically addictive, and can kill someone in one night if too much of it is taken.

Why are we ok with alcohol and not psychedelics? Because the effects are more familiar to us. We understand blind rage and reckless risk taking far more that people who say they are hearing voices. One scares us more than the other, even though it is statistically safer, because the effects are so foreign to us.

@Sean, yes it is possible to have residual effects of psychosis after a single ingestion of Salvia, particularly if you have been experimenting with other drugs previously. Lack of sleep will only exacerbate this problem. If your situation does not get better in the next day or two send us your email through the contact form and we can put you in touch with some specialists.

This one letter means nothing without further information (which I was unable to dig up). Was blood work done? Were other substances involved? So many questions but unfortunately I'm sure the narrower-minded will find only an excuse to abuse our civil rights even further.

Civil rights should trump safety every single time without exception. I must have the right to my own state of mind right up until the point that I actually cross the line into your rights -- then and only then should the government come into play.

Exiting the bathroom isn't safe (a very narrow door plus a power slam mechanism plus finger = cut off end of finger), Football is not safe (dislocated my finger playing with the aforementioned reattached end of finger in a splint), badminton is not safe (broken finger, don't ask), Skiing is not safe (I have come extremely close to tearing my right ACL), Water skiing is not safe (I split my foot open on a fall), Wet playground equipment isn't safe (broken right arm), Hiking is not safe (my friend just fell 30 feet off a cliff and messed up his leg), Golfballs aren't safe (safety tip, don't cut into one), Climbing is not safe, Mountain biking is not safe (hello ER?), Driving is not safe, my Motorcycle isn't safe (has anyone seen my spleen?)... Is there *anything* that is safe? Cottonballs? [link] Apparently not!

But neither do I want a completely safe life, it would be excruciatingly boring! I certainly take all reasonable precautions to protect myself (believe it or not) and I do absolutely everything I can to prevent my actions from bringing harm to others (hey, I made it past 40 without killing anyone).

Document the risk and move on, that is what we do with almost every other human activity, even those that are self-destructive.

I just honestly cannot fathom why these medicines made an exception. I really just kills my heart to see my country acting this way.

Be well!

@dreamdust:Schizo's are known to be set off by a WIDE range of substances.I have heard that too!

Everyone is schizo but most people are good at lying and deceiving themselves and comfortable with lying and pretending. The schizophrenic has lost the ability to lie to themselves and as a result become paranoid because they are seeing the true reality that all humans are cretins, and all cretins are liars, and liars never tell the truth even when they lie. You are lying to yourself if you believe you are not a liar. You are lying to yourself if you think you aren't a repressed schizophrenic. You've just learn to be comfortable with the lies and deception of our world.

Salvia only works on it's own receptors. Not every living human being will be 100% healthy in this area. Please note: up to 6 people every year DIE from tilting soda machines. No one died here, ever.

Okay for one, if I had an experience strong enough to change the way I normally behaved and "they" were taking me somewhere to give me drugs of their own, I would jump out of a moving vehicle too. I'd to anything to get away from those fascists.
When I took Salvia, it was 20x and I did it with the right combination of sensory deprivation + stimulation (darkness, cushions, and "P h i l o s o m a t i k a," Goa trance music). It was enough to make me act crazy for two or three weeks, when I would claim to have divine knowledge and direct contact with God... I really did believe I had seen through to the afterlife, or the other side, that it was where we came from, etc etc etc etc effectively making me a raving lunatic.
I think it just does that for a while. I do know that nobody gave me Risperidone, so I can't say what is supposed to happen when you suddenly go off of it. But I do know someone who does that now and then, and you can notice. I'm just saying, still a little skeptical here.

It would be interesting to know what they mean by "shortly after smoking Salvia." 30 seconds after smoking, for example, may have different implications that a week after smoking Salvia, both of which could theoretically be described as "shortly," depending upon the context.

It would also be interesting to know how many times (and how frequently) the patient partook. They are speculating about plastic, long-term changes brought about through the use of Salvia, so I would think that would be very relevant.

The letter is interesting and evocative, but it lacks the detail needed to come to any kind of conclusions, even provisional ones. More detail is needed to support their claims of causation and/or correlation.

Still kills/messes up a fucktonne less people a year than alcohol and tobacco.
siekostoner. : 2009-07-01 22:45:45
for about half a year straight, i smoked 20x salvia at least once a week, sometimes 2-3 times..... i was utterly fascinated with it, and as far as i know, i haven't suffered and lasting psychological effects from it. or any negative effects in general. so i'd have to say at least in my opinion, its pretty safe.

Article seems very objective. Not suprising really. Interested if they'll be more associations developing. Schizo's are known to be set off by a WIDE range of substances.

I'm quite curious about the circumstances surrounding the actual consumption of the salvia. I understand that there are some pretty damn strong extracts available, i suspect, in part due to people who do not understand the reverse tolerance effect that occurs with multiple doses, or who do not care to try it more than once.

I only have experience with lower extracts, at most "30x", but to be honest, that is more than enough.
